DIRECTIONS FOR APPLYING FOR PRIOR APPROVAL OF CONTINUING EDUCATION

These are the steps to submit information for credit approval:

STEP #1:

Obtain Continuing Professional Education Prior Approval Request (PA) Form

Note: If program is a "webinar", please do not send forms for approval to the
Wisconsin Dietetic Association, CDR handles those requests.

STEP #2:

 

The program chair needs to provide two types of information.

#1: A Completed Prior Approval (PA) Form
All necessary items must be included on the form. Incomplete PA forms may delay approval or result in denial.

  • Program title
  • Date
  • Location
  • Program Provider (organization)
  • Target audience
  • Estimated attendance.
  • Name of program chair and/or contact person, address, phone number, email address and fax number.
  • Program Information
    • The number of CPEUS requested and the CPEU level. (This information is used by the Commission on Dietetic Registration and is not judged by the WDA Continuing Education Committee.)
  • Type of activity (e.g. journal club, seminar, workshop, etc.)

#2: Program Information

  • Educational objectives and anticipated learner outcomes.
  • Name of speaker(s) and speaker qualifications. (It is best to state qualifications that relate to expertise on the topic presented. A completed curriculum vitae is not necessary.)
  • Timing outline (important - do not omit this).
    • Committee members will verify the hours you have requested. For case presentations, skill development, interactive workshops, lectures, seminars or journal clubs, etc..., 60 minutes = 1 CPE hour.
    • CPE credit is given for all appropriate educational sessions as well as questions and answers or discussion periods. It is not given for introductions/welcomes, breaks, exam time or evaluations.
    • If the total number of allowable hours includes a fraction of an hour, round to the nearest whole hour (e.g 5 1/4 hours=5 CPE hours; 5 1/2 hours= 6 CPE hours).
    • DO NOT round up or down for each session, but only after adding the total number of hours for the entire program. Exhibits and tabletops do not need prior approval but should be included in the outline.
STEP #3:

Four to six weeks before the program the final mailing should include:

  • FOUR sets of the PA form and any attachments (1 original and 3 copies)
  • Fee of $10.00 per CPE unit requested excluding any hours for exhibits and tabletop displays.

Checks are payable to the Wisconsin Dietetic Association or WDA.

STEP #4:

Follow-up

The CE chair may contact you to clarify information or recommend changes, as needed.

  • When a decision is reached, the CE chair will return your PA form clarifying approval or disapproval, a certificate of attendance that can be used and a cover letter.
  • To provide a certificate of attendance (not completion) for the participant's portfolio, you have two options.
    1. Either hand out a certificate of attendance like the one sent to you by the CE chair
    2. Print on the bottom of any handout with an agenda "This program is approved by ADA/CDR; maintain this form for your records".
